Downloading File from Custom Document Type fails when there is no ddmFormInstanceRecord

When downloading files from custom document types, download fails when there is no ddmFormInstanceRecord
In particular, issue appears when Custom Document Type includes an upload field

This is a verified issue that has been addressed in LPD-27660
